I agree to the Sewickley Community Center Rules and Regulations (required)

I acknowledge the center rules: a. No chairs or furniture of any type is permitted on the gym floor. b. No unseemly conduct (fighting, gambling, drug usage/sales, etc.) allowed on premises. c. Only appropriately soled athletic shoes are allowed on the gym floor. d. The Community Center and its property is a non-smoking environment. e. The Community Center and its property is a alcohol free environment. f. Lessee is responsible for clean up after the event.

I agree to the Waiver, Release, and Indemnification (required)

I acknowledge that the use of Sewickley Community Center facilities is voluntary. It is the express intention of the undersigned to exonerate the Sewickley Community Center from any causes of action, claims, or demands of any nature whatsoever related to injury or illness, in accordance with the terms specified via the Waiver, Release, Indemnification, & Covenant Not to Sue. This includes, but is not limited to, causes of action, claims, or demands of any nature arising from claims of negligence.

I agree to the Sewickley Community Center Photo and Website Waiver and Release

I grant The Center the absolute and irrevocable right and unrestricted permission concerning any photographs that he/she has taken or may take of me or my minor in which I/they/we may be included with others, to use, reuse, publish, and republish the photographs in whole or in part, individually or in connection with other material, in any and all media now or hereafter known, including the internet, and for any purpose whatsoever, specifically including illustration, promotion, art, editorial, advertising, and trade, without restriction as to alteration. I release and discharge the Photographer from any and all claims and demands arising out of or in connection with the use of the photographs, including without limitation any and all claims for libel or violation of any right of publicity or privacy. This authorization and release shall also benefit the heirs, legal representatives, licensees, and assigns of the Photographer and the person(s) for whom he/she took the photographs. I am a legally competent adult and have the right to contract in my own name. I have read this document and fully understand its contents. This release shall be binding upon me and my heirs, legal representatives, and assigns.
